Job summary

Elevance Health is seeking a LTSS Service Coordinator-RN Clinician (Case Manager) who will coordinate and manage members' care across physical health, behavioral health, and long-term services. This field-based role requires Ohio residency and involves travel to client sites with occasional office attendance for meetings or training.

The RN will develop, monitor, and revise individualized care plans, prioritizing person-centered care, while coordinating with Medical Directors and the health care

Qualifications

• Requires a high school diploma or GED and at least 3 years' experience with chronic illnesses in a service coordination or case management role.
• Current, active, valid RN license in Ohio.
• BSN/BS in Nursing preferred; strong case management experience with older adults or disabilities is noted.

Responsibilities

• Perform telephonic and face-to-face assessments to identify, coordinate, and manage member needs across physical health, behavioral health, and long term services.
• Manage care for members with chronic illnesses to ensure cost-effective and efficient utilization of health benefits.
• Obtain thorough member history to develop individualized care plans with short and long-term goals.
• Collaborate with members, caregivers, family, and physicians to determine appropriate care levels and alternative options.
• Develop and monitor care plans; facilitate authorizations/referrals within benefits or through extra-contractual arrangements.
