Job summary

AdventHealth Orlando is seeking an experienced registered nurse to join our care management team. You will participate in multidisciplinary rounds, assess discharge needs, and coordinate post-acute services to ensure timely, safe transitions for patients.

You will work to educate patients and families about care plans, resource needs, and community supports, while communicating effectively with payors and care teams across the continuum.

Qualifications

• Associate's degree in Nursing (RN) required.
• Bachelor's degree in Nursing preferred.
• 2+ years of medical/hospital nursing experience required.
• RN license required.
• CCM/ACM certifications preferred.

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families about illness, financial/resource implications, and discharge options.
• Assess readmitted patients and coordinate post-acute care planning.
• Document discharge planning evaluations, barriers, and care progression.
• Coordinate with payors for post-acute care authorization as needed.
