AdventHealth in Daytona Beach, FL seeks an RN with discharge planning and care coordination responsibilities. You will participate in rounds, educate patients and families, and ensure timely transitions to post-acute services. This role requires RN licensure and 2+ years of medical/nursing experience.

Strong communication, organization, and data-analysis skills are essential. PRN day shifts and a comprehensive benefits package are offered to support whole-person care.

Qualifications

• Associate's degree in Nursing required.
• Registered Nurse (RN) license required.
• Bachelor's degree in Nursing preferred.
• Minimum 2+ medical/hospital nursing experience.
• Prior Care Management/Utilization Management experience preferred.

Responsabilités

• Participates in multi-disciplinary rounds to review patient status, discharge plans, and resource needs.
• Educates patients and families on illness impact and post-acute care resources.
• Assesses readmissions and discharge needs.
• Organizes patient and family care conferences with the multidisciplinary team.
• Documents discharge planning evaluations and progression of care.
• Communicates with payors for post-acute care authorization.
• Develops discharge plans with contingency arrangements for post-acute care.
