Job summary

Qualifications

• Requires RN licensure and related nursing qualifications.
• Associate's degree in nursing required; Bachelor's preferred.
• 2+ years of medical/hospital nursing experience required.

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families on emotional, social, and financial impacts of illness and mobilizes resources.
• Develops discharge plans with contingency plans to ensure timely care coordination.
• Communicates with payors for post-acute care authorization as needed.
• Documents discharge planning evaluations and barriers to progression of care.
