Job summary

AdventHealth Winter Garden in Florida seeks an experienced Registered Nurse to support discharge planning and care coordination. You will participate in rounds, educate patients and families, and assist with post-acute arrangements to ensure timely, safe transitions.

A strong background in nursing and utilization management is preferred. The role requires leadership, adaptability, and excellent communication as you partner with diverse teams and community resources to optimize patient outcomes

Qualifications

• RN license required.
• Associates degree in Nursing required.
• Bachelor's degree preferred.
• 2+ years of medical/hospital nursing experience required.
• Care Management/Utilization Management experience preferred.

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status, progression and discharge plans.
• Educates patients and families on illness impact and mobilizes resources for needs.
• Assess readmissions and understand reasons.
• Organize and facilitate patient/family care conferences with the team.
• Document discharge planning evaluations and communicate needs for post-acute care with Payors.
