Job summary
AdventHealth in Daytona Beach, FL is hiring for a full-time discharge planning RN to coordinate care across multidisciplinary teams and ensure smooth transitions from hospital to post-acute services. You will educate patients and families about care plans, assess readmission risks, and develop comprehensive discharge plans that align with CMS requirements.
The role requires RN licensure, a nursing degree, and 2+ years in medical/hospital nursing with strong leadership and communication skills.
Qualifications
- RN license required.
- 2+ medical/hospital nursing experience.
- Care Management/Utilization Management experience preferred.
- Bachelor’s degree in Nursing preferred.
Responsibilities
- Actively participates in multi-disciplinary rounds to review patient status, progression and discharge plans.
- Communicates with patients and families about emotional, social, and financial impacts of illness.
- Develops discharge plans and coordinates post-acute care services to ensure timely care transitions.
- Reviews medical records and integrates clinical, social, and financial factors into discharge planning.
