Job summary

AdventHealth in Florida seeks an experienced RN for discharge planning and case management. You will coordinate post-acute care, educate patients and families, and document outcomes to ensure timely transitions across the care continuum.

Required is an active RN license with an Associate's degree; a Bachelor's is preferred. This PRN role offers benefits from day one and opportunities for growth within the organization.

Qualifications

RN with an Associate's Degree in Nursing is required.
Bachelor's Degree in Nursing is preferred.
Florida RN license is required.

Responsibilities

Leads discharge planning and coordinates post-acute care services.
Participates in multidisciplinary rounds to review patient status and discharge plans.
Educates patients and families about care decisions, resources, and transitions.
Documents discharge plans and progress in medical records and related systems.
