Qualifications

• Must hold a Florida RN license with at least 2 years of hospital nursing experience.
• Care management/utilization management experience is preferred.
• Knowledge of discharge planning and post-acute care processes.
• Familiarity with CMS Conditions of Participation for Discharge Planning.
• Strong communication with patients, families, and care teams.
• Proficient use of EMR systems and Outlook for care coordination.

Responsibilities

• Actively participates in multidisciplinary rounds to review status and discharge plans.
• Educates patients and families about illness impacts and post-discharge resources.
• Assesses readmissions and plans timely transitions of care.
• Organizes and facilitates family care conferences with the team.
• Documents discharge planning evaluations, assessments, and barriers.
• Coordinates with post-acute services and payors for authorizations.
• Ensures timely care coordination and complete medical records.
