Job summary

AdventHealth in Orlando, FL is seeking an experienced Registered Nurse to participate in discharge planning and care management across hospital units. You will collaborate with multi-disciplinary teams to assess patient needs, coordinate post-acute services, and ensure timely transitions.

Required: RN license, Associates degree in Nursing; 2+ years medical/hospital nursing; CCM/ACM preferred; BLS preferred.

Qualifications

• RN license required (Florida)
• 2+ years of medical/hospital nursing experience required
• CCM/ACM certification preferred
• BLS certification preferred

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families on illness, resources, and care decisions.
• Assesses readmission reasons and formulates discharge plans.
• Organizes patient and family care conferences with the team.
• Documents discharge planning activities and communicates with payors for post-acute needs.
• Reviews medical records to incorporate clinical and social factors into care transition.
• Develops contingency plans for timely progression of care and post-acute arrangements.
• Leverages technology to ensure complete records and discharge reconciliation.
