Job summary

Orlando Health is seeking an experienced RN Care Coordinator to collaborate with clinical teams and identify patients who can benefit from coordinated care across inpatient, emergency, and ambulatory settings.

You will lead discharge planning, engage patients and families, educate on self-management, and ensure access to medications and DME. The role requires a Florida RN license, BSN, and a track record in care coordination.

Qualifications

• RN licensed in Florida with BSN.
• National certification in specialty area within 2 years.
• Current BLS; ACLS/PALS/NALS/TNCC as required.
• Three years of direct clinical experience in relevant population.

Responsibilities

• Lead continuity of care across inpatient, ER, and ambulatory to ensure integrated delivery.
• Develop relationships with care team to engage patients and families.
• Monitor discharge plans and adjust as patient condition changes.
• Educate patients and families on self-management and care systems.
• Facilitate access to prescriptions, DME, and other services.
• Promote patient safety and comply with policies.
