The ideal candidate holds a Bachelor of Science in Nursing, has 5 years of direct patient care, and is proficient in Microsoft Office and Epic.

Qualifications

• Five years of clinical direct patient care experience
• Master's degree in healthcare-related field is preferred
• Proficiency with Microsoft Office and Epic systems

Responsibilities

• Coordination of applicable program(s) and leadership in patient care across the continuum.
• Oversee program operations, quality improvement, and accreditation activities.
• Collaborate with physicians, nursing, case management, and other health professionals to ensure high-quality, cost-efficient care.
