Job summary

TheKey is seeking a Client Success Manager for Vienna, VA and surrounding area. This full-time, exempt role blends field visits with office work to support seniors and their families through onboarding, care planning, and ongoing coordination.

You will lead clinical intake, develop individualized care plans, supervise caregivers, ensure regulatory compliance, and maintain documentation in ClearCare/WellSky, while building strong relationships with clients and local healthcare partners.

Qualifications

• Active RN license in Virginia is required.
• Associate Degree in Nursing is required; BSN preferred.
• 5+ years in healthcare, geriatrics, home health, hospice or case management.
• Experience with electronic health records / home-care software (ClearCare/WellSky a plus).

Responsibilities

• Lead initial clinical intake and ongoing assessments.
• Craft individualized care plans and ensure service delivery.
• Partner with clients, families, and staff to provide high-touch support.
• Remain compliant with state regulations and clinical standards.
• Maintain timely documentation in ClearCare/WellSky.
• Identify cross-referral opportunities and coordinate LTCI processes.
• Participate in after-hours on-call triage when needed.
