TheKey is seeking a Client Success Manager for Vienna, VA and surrounding areas. This RN-led role combines in-home assessments with care coordination in a hybrid field/office setting. Travel up to 30% within the local area supports client and family needs.

You will develop and adjust care plans, mentor caregivers, and ensure high-quality service delivery while navigating documentation in ClearCare/WellSky and coordinating with local providers.

Qualifications

• Active RN license in Virginia.
• Associate Degree in Nursing required; BSN preferred.

Responsabilités

• Lead initial clinical intake and ongoing assessments.
• Develop individualized care plans based on health assessments and routines.
• Supervise and mentor caregivers to ensure regulatory compliance.
• Conduct regular home visits and quality checks to support retention.
• Maintain accurate documentation in ClearCare/WellSky and coordinate with staff.
• Identify referral opportunities and strengthen community partnerships.
