TheKey, LLC is seeking an RN Client Success Manager in the Manhattan, NY area for a hybrid field/office role. You will lead initial clinical intake and ongoing assessments, craft individualized care plans, and ensure exceptional service delivery to seniors and their families.

Formación

• Active NY RN license.
• Associate Degree in Nursing required; BSN preferred.
• 5+ years in healthcare, geriatrics, home health, hospice, case management or social services.
• Exceptional bedside manner and communication skills.
• Experience with electronic health records/home-care software (ClearCare/WellSky) is a plus.
• Valid driver's license and reliable vehicle for local travel in NY area.

Responsabilidades

• Conduct in-person nursing assessments and develop care plans for new and existing clients in Manhattan, NY area.
• Convert assessments into dynamic, customized care plans and adjust as conditions change.
• Supervise caregivers to ensure compliance with state home care regulations and clinical standards.
• Perform regular home visits and quality checks to support retention and service quality.
• Maintain timely documentation in ClearCare/WellSky and collaborate with staffing coordinators.
• Identify cross-referral opportunities and strengthen partnerships with local healthcare providers.
• Participate in rotating after-hours/on-call triage as needed.
