TheKey Of Virginia LLC is seeking an experienced RN Client Success Manager to ensure high-quality, personalized care for clients at home. You will create and reassess care plans, monitor service delivery, and partner with families and teams to optimize client outcomes.

Responsibilities include coordinating care with caregivers, documenting plans, and maintaining compliance with state regulations while supporting long-term client retention.

Qualifications

• Current Registered Nurse license in the state of employment.
• 5 years experience in health care, elder care, social work, or related industry preferred.
• Excellent customer service and conflict resolution skills.
• Computer proficiency and ability to document accurate and timely notes in systems related to client visits.
• Current driver’s license and proof of insurance.

Responsibilities

• Manage the day-to-day client relationship and delivery of quality care.
• Identify and focus on a holistic approach to client care with improved quality of life.
• Conduct in-person nursing initial and reassessments and quality assurance visits with Care Plans.
• Ensure care team stays up-to-date and care plans are documented and updated.
• Collaborate with internal and external customers through regular meetings.
