Qualifications

• Active RN license in Virginia is required.
• Associate Degree in Nursing required; BSN preferred.
• 5+ years in healthcare, geriatrics, home health, hospice, case management, or social services preferred.
• Excellent communication and interpersonal skills.
• Experience with electronic health records and home-care software (ClearCare/WellSky a plus).

Responsibilities

• Conduct comprehensive in-person nursing assessments and home safety reviews.
• Develop and update individualized care plans based on assessments.
• Guide and supervise caregivers to ensure regulatory and clinical standards.
• Perform regular home visits and quality assurance checks for retention.
• Maintain documentation in ClearCare / WellSky and coordinate with staff.
• Identify referral opportunities and strengthen partnerships.
• Participate in after-hours on-call triage as needed.
