Job summary

TheKey is hiring an RN Client Success Manager to lead clinical care for homebound seniors. This full-time role involves in-person assessments, care planning, and mentoring caregiving teams, with local field travel up to 75%.

You will ensure regulatory compliance, advocate for clients and families, and coordinate with interdisciplinary partners and payors to deliver high-quality, person-centered care.

Qualifications

• Active, unencumbered RN license in state of employment (BSN preferred).
• 5+ years in healthcare, elder care, home health, or case management preferred.
• Strong clinical judgment, interpersonal and conflict resolution skills; tech-savviness for electronic documentation.
• Valid driver's license, auto insurance, and ability to travel locally up to 75%.

Responsibilities

• Conduct in-person clinical assessments, home safety evaluations, and routine reassessments.
• Build relationships with clients and families to ensure satisfaction and quality of life.
• Lead, mentor, and supervise home caregivers to ensure safety and service quality.
• Ensure compliance with state regulations and clinical best practices.
• Oversee care escalations and coordinate with interdisciplinary partners and payors.
