Job summary

HCA Florida Westside Hospital is seeking a Registered Nurse Case Manager to oversee rehabilitative patient care and coordinate interdisciplinary treatment plans. The CM will work with the Rehab Program Director and Facility Case Management Director to align departmental functions and ensure high-quality outcomes.

The ideal candidate has an active RN license, 3+ years of acute care nursing and experience or certification in case management.

Qualifications

• RN with current state licensure; BSN preferred.
• Three years’ clinical acute care hospital nursing experience; preference for 2 years in case management or 2 years of critical care.
• Certification in case management or utilization review preferred.
• InterQual experience preferred.

Responsibilities

• Provide program orientation to patients/families/caregivers and explain case manager role, Rehab philosophy, discharge criteria, Medicare benefits and rights.
• Coordinate education of patient/family/caregiver through Family Conferences and team discussions.
• Ensure implementation of treatment plans that align with patient strengths and needs; facilitate patient involvement in rehabilitation.
• Document Discharge Planning Evaluation and psychosocial assessments; communicate social, financial, or discharge needs and preferences.
• Promote consistent, positive patient interactions and unparalleled patient service.
