Job summary

HCA Florida Mercy Hospital in Miami seeks an experienced RN with case management expertise to coordinate rehabilitation services. You will guide patients and families through orientation, discharge planning, and goals discussions, and collaborate with the care team to ensure patient-centered care.

Required Florida RN license; BSN preferred. Three years of clinical hospital nursing with preference for case management or critical care experience.

Qualifications

• RN with current state licensure; BSN preferred.
• Three years’ clinical, hospital nursing experience required with preference for 2 years of case management or 2 years of critical care experience.
• Certification in case management or utilization review preferred.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Develop and implement individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team.
• Collaborate closely with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction.
