Job summary

HCA Florida Woodmont Hospital in Florida is seeking a Registered Nurse Case Manager to coordinate rehabilitative care and ensure patient-centered outcomes. You will work closely with the Rehab Program Director and Facility Case Management Director to align care plans with discharge criteria and payer requirements.

You will lead patient/family education, document assessments, and promote seamless collaboration across the rehab team to deliver outstanding service and timely, appropriate care.

Qualifications

• RN has current state licensure.
• Minimum 3 years of clinical acute care hospital nursing experience.
• Certification in case management or utilization review preferred.
• InterQual experience preferred.

Responsibilities

• Provide program orientation to patients/families/caregivers, including the case manager's role and discharge criteria.
• Coordinate education and involve patient/family in plans and status via Family Conferences and other interactions.
• Ensure implementation of the treatment plan addressing patient strengths, abilities, needs, and preferences.
• Document findings from the Discharge Planning Evaluation and psychosocial assessments.
• Promote consistent, positive patient interactions and exceptional service.
