HCA Florida Lawnwood Hospital in Florida is seeking a Registered Nurse Case Manager to coordinate rehabilitative care and ensure patient-centered outcomes. You will work with the Rehab Program Director and care team to align plans with patient needs and discharge goals.

The role emphasizes patient and family education, discharge planning, and documentation of psychosocial assessments, with priority given to those with case management or critical care experience and Florida RN licensure.

Qualifications

• Active RN license in Florida (state licensure required).
• BSN preferred.
• Minimum 3 years of acute care hospital nursing experience.
• Preference for 2 years in case management or 2 years in critical care.
• Certification in case management or utilization review preferred.
• InterQual experience preferred.

Responsibilities

• Provide program orientation to patients/families/caregivers.
• Coordinate patient/family/caregiver education and participate in team conferences.
• Ensure implementation of the treatment plan and involve patient throughout rehab.
• Document Discharge Planning Evaluation and psychosocial assessments.
• Promote positive patient interactions and exceptional service.
