Job summary

HCA Florida Woodmont Hospital is seeking an experienced Registered Nurse Case Manager to coordinate rehabilitative care and ensure optimal patient outcomes. You will work with the Rehab Program Director and Facility Case Management Director to align care plans, discharge planning, and education for patients and families.

The role requires an active RN license, 3+ years of acute care hospital nursing, and preferred certification in case management with InterQual experience.

Qualifications

• RN with current state licensure
• Three years’ clinical acute care hospital nursing experience required
• Certification in case management or utilization review preferred
• InterQual experience preferred

Responsibilities

• Provide program orientation to patients/families/caregivers and explain case management role, rehab philosophy, and discharge criteria
• Coordinate education with patient/family/caregiver and participate in team discussions through conferences
• Ensure implementation of the patient’s treatment plan and involve the patient throughout rehabilitation
• Document discharge planning evaluation and psychosocial assessments and communicate needs and preferences
• Promote consistent, high-quality patient interactions and unrivaled patient service
