Job summary

HCA Florida Highlands Hospital in Sebring, FL is seeking an RN Case Manager (PRN) to coordinate rehabilitative care and ensure quality, patient-centered outcomes. You will align with the Rehab Program Director and Facility Case Management Director, guiding discharge planning, education, and care plans within a multidisciplinary team.

The role requires an active RN license, 3+ years of acute hospital nursing experience, and preference for case management or critical care background.

Qualifications

• RN with current state licensure; BSN preferred.
• Three years’ clinical acute care hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience
• Certification in case management or utilization review preferred
• InterQual experience preferred

Responsibilities

• Provide program orientation to patients/families/caregivers. Include case manager role, Rehab philosophy, discharge criteria, Medicare/insurance benefits, rights and responsibilities.
• Coordinate patient/family/caregiver education and promote participation in team discussions.
• Ensure implementation of patient treatment plans and involvement in rehabilitation process.
• Document Discharge Planning Evaluation and psychosocial assessments, communicate social/financial/discharge needs.
• Promote consistent, positive patient interactions and unparalleled patient service.
