Job summary

HCA Florida Highlands Hospital in Sebring, FL seeks a Case Manager (RN) to coordinate rehabilitative care and ensure patient-centered outcomes. You will collaborate with the Rehab Program Director and discharge teams to tailor plans for each patient.

The role requires an active RN license, with BSN preferred, plus 3 years of acute hospital nursing and 2 years in case management or critical care. InterQual and CM certification are a plus.

Qualifications

• RN with current state licensure; BSN preferred.
• Three years’ acute care hospital nursing experience required.
• 2 years of case management or 2 years of critical care preferred.
• Certification in case management or utilization review preferred.

Responsibilities

• Provide program orientation to patients/families/caregivers and explain plans, benefits, and rights.
• Coordinate patient/family education and involve them in team discussions.
• Ensure implementation of the patient’s treatment plan and promote engagement.
• Document Discharge Planning Evaluation and psychosocial assessments and needs.
• Promote consistent, positive patient interactions and service excellence.
