Job summary

HCA Florida Gulf Coast Hospital is seeking a dynamic RN Case Manager for Inpatient Rehab. You will assess psychosocial and medical needs, advocate for patients, coordinate the plan of care, and liaise with physicians and families to ensure efficient patient throughput and appropriate resource use.

Requirements include an active Florida RN license (BSN preferred), BLS-AHA certification within 30 days, and 3 years of acute care RN experience.

Qualifications

• RN license required; Florida license or multistate with 60 days to obtain Florida license after relocation.
• BLS-AHA Provider card required within 30 days of start.
• 3 years of RN experience in an acute care setting required.
• Prior experience in Case Management preferred.
• BSN preferred.
• Certification in Case Management, Nursing or Utilization Review preferred.

Responsibilities

• Performs comprehensive psychosocial and medical assessments.
• Advocates for patients and leads the interdisciplinary team.
• Coordinates plan of care.
• Communicates with physicians, patients, families, staff and outside agencies.
• Performs utilization management reviews and communicates with third-party payors.
• Facilitates patient throughput with focus on quality and efficiency.
