Job summary

HCA Florida Woodmont Hospital is looking for a Case Manager to support rehabilitative care and coordinate patient progress from admission to discharge. The CM collaborates with the Rehab Program Director and Facility Case Management Director to ensure high‑quality care tailored to each patient’s needs.

You will orient patients/families, coordinate education, document evaluations, and communicate discharge needs while promoting exceptional patient service across the rehab team.

Qualifications

• RN with current state licensure.
• Three years’ clinical acute care hospital nursing experience required; case management or 2 years of critical care preferred.
• Certification in case management or utilization review preferred.
• InterQual experience preferred.

Responsibilities

• Provide program orientation to patients/families/caregivers and explain case manager roles, Rehab philosophy, discharge criteria, and benefits.
• Coordinate education and involve patient families in plans, goals, and status through conferences and interactions.
• Ensure implementation of the treatment plan aligned with patient needs and promote involvement throughout rehabilitation.
