Job summary

Ideal candidates hold Florida RN licensure with a BSN preferred, and three years of acute hospital experience. Certification in case management and InterQual experience are a plus.

Qualifications

• RN licensed in Florida with current credentials.
• Three years of acute care hospital nursing required.
• Case management certification preferred; InterQual experience preferred.

Responsibilities

• Provide program orientation to patients, families, and caregivers about rehab plans and rights.
• Coordinate patient/family education through conferences and team discussions.
• Ensure implementation of treatment plans aligned to patient needs and preferences.
• Document Discharge Planning Evaluation and psychosocial assessments.
• Promote positive patient interactions and unparalleled service.
