Job summary

Vibra Healthcare is seeking an RN Case Manager to integrate case management into patient care, discharge, and home planning. You will coordinate with hospital departments and external agencies to support patients and families.

The role requires a Bachelor’s in a healthcare field or RN licensure, and two years’ RN or social work experience is preferred. You will ensure PHI protection and participate in quality improvement initiatives.

Qualifications

• Bachelor's degree in a healthcare related field or RN licensure.
• Two years’ healthcare/hospital experience as RN or Social Worker preferred.
• Licensed as an RN or Social Worker in state of employment; BLS certification within 30 days.

Responsibilities

• Directs and coordinates case management within patient care, discharge, and home planning.
• Perform psychosocial assessments and develop care plans with families and care teams.
• Coordinate post-discharge services with community and home health agencies.
• Collaborate with physicians, nurses, and management to plan patient care.
• Maintain PHI protections and participate in performance improvement activities.
