CHI St. Vincent Health is seeking an RN Case Manager to lead discharge planning, coordinate care across the continuum, and advocate for patients through transitions with physicians, nursing, insurers, and post-acute providers.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum two (2) years of acute hospital clinical experience.
• Master's in Case Management or Nursing.
• Registered Nurse license in AR.
• Basic Life Support - CPR within 90 days.

Responsibilities

• Completes and documents a discharge planning assessment.
• Facilitates development of multidisciplinary discharge plan with team and post-acute providers.
• Oversees and evaluates implementation of the discharge plan.
• Collaborates with multidisciplinary team to ensure progression of care and appropriate inpatient resource use.
• Collaborates with healthcare team and post-acute service providers for timely transitions to post-acute services.
• Identifies risk for readmission and implements interventions for at least 30 days.
