• Bachelor's degree in a healthcare related field, social service field, or RN.
• Licensed as a Registered Nurse or Social Worker in state of employment.
• Knowledge of basic computer skills.

• Directs and coordinates integration of Case Management into patient care, discharge, and home planning processes.
• Coordinate clinical care for patients, families to maximize health care benefits.
• Complete psychosocial assessment on all new admissions within three business days.
• Develop and implement programs to assist patients and families with social/psychological factors of illness.
• Plan patient care services with physicians and nursing staff; participate in adherence and education efforts.
• Coordinate discharge and post-hospital care with community service providers.
• Provide information and guidance to patients and families regarding post-discharge treatment and recovery programs.
• Coordinate referrals to community, social services, home health, extended care, and rehabilitation facilities.
