Vibra Healthcare in Paducah, KY, seeks an RN Case Manager to coordinate case management across the care continuum, discharge planning, and home health planning, collaborating with hospital departments and external partners.

The role requires a Bachelor's degree in a healthcare-related field or RN, with 2 years of healthcare/hospital experience preferred; active RN or Social Worker licensure in the employment state is required, and BLS within 30 days.

Competenze

• Bachelor's degree in a healthcare related field, social service field, or RN.
• Two years’ healthcare/hospital experience as RN or Social Worker preferred.
• Licensed as a Registered Nurse or Social Worker in state of employment; BLS within 30 days of employment.

Mansioni

• Directs and coordinates the integration of Case Management into patient care, discharge, and home planning processes.
• Provide care for the Adult or Geriatric patient, ages 18 years and over.
• Coordinates clinical care to enable patients and families to cope with illness and maximize health care benefits.
• Complete psychosocial assessment on new admissions within three business days.
• Develop and implement programs to assist patients and families with social and psychological factors of illness.
• Plan patient care services with physicians, nursing staff, and management.
