Vibra Healthcare is seeking an RN Case Manager in Kentucky to direct and coordinate case management within patient care, discharge, and home planning processes. You will collaborate with hospital staff and external organizations to optimize outcomes and ensure appropriate post-discharge care.

Responsibilities include psychosocial assessments, care planning with physicians, and discharge coordination. Requires RN license, BLS, and 2 years healthcare/hospital experience preferred.

• Education/Experience:
• Bachelor's degree in a healthcare related field, social service field, or RN.
• Two years’ Healthcare/Hospital experience as RN or Social Worker preferred.
• Computer Skills:
• Knowledge of basic computer skills and word processing.
• Certificates and Licenses:
• Licensed as a Registered Nurse or Social Worker in state of employment. Must obtain BLS within 30 days of employment and maintain for the duration of employment.

Mansioni

• Provide care for the Adult or Geriatric patient, ages 18 years and over.
• Coordinates the provision of clinical care to patients, families and significant others to enable them to deal with the impact of illness on family functioning.
• Complete psychosocial assessment on all new admissions within three business days.
• Develop and participate in programs to assist patients and families with social and psychological factors of illness.
• Develop with physicians, nursing personnel, and others in planning of patient care services.
• Engage in individual or group intervention sessions pertaining to care and adjustment.
• Arrange for discharge and post-hospital care through community resources.
• Provide advice and guidance in handling special cases or patient needs.
• Coordinate information and guidance to patients and families regarding post-discharge treatment and recovery.
• Coordinate referrals to community, social service, home health, and rehab facilities.
• Serve as liaison with external social service organizations.
• Participate in interdepartmental program planning and Case Management education.
• Represent the department on hospital committees.
• Develop and implement Pastoral Care Program with Host hospital.
• Maintain harmonious relationships with other departments and address social service issues.
• Assist in budget preparation and monitoring; monitor utilization reviews and patient LOS.
• Direct quality improvement activities and prepare department reports.
