Qualifications

• Licensure as a Registered Nurse or Social Worker in the state of employment; BLS must be obtained within 30 days.

Responsibilities

• Coordinate clinical care planning with physicians and nursing staff to develop patient care plans.
• Complete psychosocial assessment on new admissions within three business days.
• Coordinate discharge planning and post-hospital care with community resources and home health services.
