Job summary

Vibra Healthcare is seeking an experienced RN Case Manager to direct and coordinate case management within the patient care and discharge planning process. The role includes psychosocial assessments, collaboration with physicians and nursing staff, and ensuring seamless post-discharge planning with community resources.

The position emphasizes coordination of care for adult and geriatric patients, development of support programs, and active involvement in hospital committees to improve outcomes.

Qualifications

• Bachelor's degree in a healthcare related field, social service field, or RN.
• Two years’ Healthcare/Hospital experience as RN or Social Worker preferred.
• Knowledge of basic computer skills and word processing.
• Licensed as a Registered Nurse or Social Worker in state of employment. Must obtain BLS within 30 days of employment and maintain for the duration of employment.

Responsibilities

• Directs and coordinates the integration of Case Management into patient care, discharge, and home planning.
• Provide care for Adults and Geriatric patients (18+).
• Coordinate clinical care for patients and families to maximize benefits from health services.
• Complete psychosocial assessment on new admissions within three business days.
• Develop programs to assist patients and families with psychosocial factors of illness.
• Plan with physicians and nursing staff in patient care services and care plan.
• Coordinate discharge and post-hospital care through community resources.
• Coordinate case management communications and act as liaison with external social services.

Education

Bachelor's degree in a healthcare related field, social service field, or RN

Licensed as a Registered Nurse (RN) or Social Worker in state of employment
