Vibra Healthcare Illinois is seeking an RN Case Manager to Direct and coordinate integration of Case Management into patient care, discharge, and home planning. This role involves working with hospital departments and external organizations to optimize patient outcomes.

Qualifications include a bachelor's degree in a healthcare-related field or RN, RN or Social Worker licensure, and BLS within 30 days. Two years of relevant healthcare/hospital experience is preferred.

Competenze

• Bachelor's degree in a healthcare related field, social service field, or RN.
• Two years’ Healthcare/Hospital experience as RN or Social Worker preferred.
• Licensed as a Registered Nurse or Social Worker in state of employment. Must obtain BLS within 30 days of employment.

Mansioni

• Directs and coordinates integration of Case Management into patient care, discharge, and home planning processes.
• Complete psychosocial assessment on all new admissions within three business days.
• Develop and implement programs to assist patients and families with social and psychological factors of illness.
