Vibra Healthcare in Kentucky is seeking an experienced RN Case Manager to coordinate patient care, discharge planning, and home care arrangements. The role requires collaboration with physicians and nursing staff to develop and implement care plans that address medical and psychosocial needs.

A valid RN license and BLS certification are required, with preference for those having hospital case management experience.

Competenze

Bachelor's degree in a healthcare related field, social service field, or RN.
Two years’ healthcare/hospital experience as RN or Social Worker preferred.
Licensed as a Registered Nurse or Social Worker; BLS certification within 30 days.

Mansioni

Direct care for adults or geriatrics 18+, coordinating clinical care with patients, families and teams.
Perform psychosocial assessments on new admissions within three days.
Develop and implement programs to address social and psychological factors of illness.
Collaborate with physicians, nursing staff and management on patient care planning.
Coordinate discharge and post-hospital care with community resources.
Provide information and guidance to patients and families about post-discharge treatment and recovery.
Coordinate referrals to community, social services, home health, and rehabilitation facilities.
Contribute to interdepartmental program planning and physician education about medical necessity.
Support Pastoral Care and maintain harmonious relationships with departments.
