Competenze

• Bachelor's degree in healthcare, social work, or nursing.
• Two years of healthcare/hospital experience preferred.
• Licensed RN or Social Worker in the state of employment; BLS within 30 days.
• Knowledge of PHI handling and HIPAA compliance.

Mansioni

• Directs and coordinates case management within patient care, discharge, and home planning processes.
• Complete psychosocial assessments for new admissions within three business days.
• Coordinate care planning with physicians, nurses, and management for patient care services.
• Arrange discharge and post-hospital care through community and home health services.
• Provide guidance to patients and families on treatment and recovery options.
