Job summary

Chapters Health is seeking an RN Case Manager in Florida to assess patient and family needs, coordinate a Plan of Care with the IDT, and provide clinical, palliative and supportive care to keep patients in their homes as long as possible.

You will work with an interdisciplinary team to educate families, coordinate care, and ensure safety and comfort, including required home visits and adherence to essential job functions.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• PACE employees: Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.
• Experience with an EMR/EHR system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate in English (verbal and written).
• Ability to visit Participant in their homes to assessments.
• Ability to perform the essential functions and physical requirements (including lifting) with or without accommodations.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT with appropriate nursing intervention and follow-up.
• Coordinates the Plan of Care reflecting the patient’s evolving needs.
• Educates patient, family, caregivers and other health professionals about disease, palliative interventions, care giving and safety.
• Home visits to assess home safety, medication and DME compliance to live safely in the community.
• Reports changes in patient condition to IDT or other health professionals.
• Participates with the IDT to evaluate hospice referrals/admissions for level of care appropriateness.
• Attends daily IDT collaboration meetings and presents concise reports to IDT.
• Communicates clearly to physicians, staff, patients and families; supports team collaboration.
• Supervises patient care by CHWs and HHAs as requested.
• May be required to report to work during emergencies to ensure continuity of services.
• Performs other duties as assigned.
