Job summary

Chapters Health System seeks an RN Case Manager to assess patient needs, coordinate Plans of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to patients in their homes, helping them stay in familiar surroundings whenever possible.

Candidates must hold an active RN license, be able to visit patients in their homes, and have EMR/EHR experience. The role includes a sign-on bonus and mileage reimbursement as part of the employment package.

Qualifications

• Current license as RN in the state where the employee will be working.
• Active BLS certification for healthcare professionals.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patients and families about disease processes, palliative interventions, and safety practices.
• Conducts home visits to assess safety, medication and nutritional compliance, and DME needs.
