Job summary

Chapters Health System in Florida is seeking an RN, Case Manager to assess patient needs, coordinate the Plan of Care with the Interdisciplinary Team, and deliver clinical, palliative, and supportive care to keep patients in their homes.

The role emphasizes compassionate communication, home visits, documentation, and collaboration with physicians and IDT. Eligible for sign-on bonus and Charter School eligibility; benefits begin day one.

Qualifications

• Current RN license in the practicing state.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver's license and auto insurance per policy.
• Reliable transportation for visits.
• Ability to communicate in English (verbal and written).

Responsibilities

• Provides direct care to patients and families with IDT involvement.
• Coordinate Plan of Care reflecting evolving patient needs.
• Educate patients and families about disease, palliation and safety.
• Perform home visits to assess safety, medication/nutrition compliance.
• Document and communicate changes to IDT; contribute to referrals.
• Attend IDT meetings and relay concise reports to professionals.
