Chapters Health System is seeking a Registered Nurse, Case Manager to assess patient/family needs, coordinate the Plan of Care with the Interdisciplinary Team, and deliver clinical, palliative and supportive care to keep patients in their homes where possible.

The role requires active Florida RN license, one year of nursing experience (hospice/hospital preferred), and strong EMR/EHR skills with valid driver’s license for home visits. Expect collaboration with IDT and on-call considerations.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year nursing experience; hospice or hospital preferred.
• Active BLS certification from AHA or Red Cross.
• Experience with EMR/EHR systems.
• Valid driver’s license and reliable transportation.

Responsabilités

• Provides direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care ensuring it reflects evolving patient needs.
• Educates patient and family about disease process, palliation, safety and care.
• Home visits to assess safety, medication and adherence to plan of care.
• Communicates with physicians and IDT; participates in quality improvement activities.
