Job summary

Chapters Health System is seeking a dedicated RN Case Manager in Florida to assess patient needs, coordinate care with the IDT, and provide clinical and palliative support in the patient’s home environment.

Responsibilities include home visits, Plan of Care development, educating families, and communicating effectively with physicians and the IDT. Requires active RN license in Florida and BLS certification.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one (1) year nursing experience in hospice or hospital experience preferred.
• Experience working with an EMR/EHR system.
• Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to visit Participant in their homes to assessments.
• Ability to perform the essential functions and physical requirements of the job.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient and family about disease, prevention, palliative interventions, and safety practices.
• Reports changes in patient’s condition to IDT or other health professionals.
• Participates with IDT to evaluate referrals/admissions for level of care relevance.
• Communicates with physicians, staff, patients and families; SBAR documentation.
• Supervises home health aides as requested.
• May be required to report to work during emergencies to ensure continuity of services.
