Job summary

Chapters Health System is seeking an RN Case Manager to assess patient/family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide palliative and supportive nursing care. The role includes home visits, documentation, and collaboration with physicians and facilities to ensure quality patient outcomes.

The candidate should have an active RN license in Florida, at least one year of nursing experience (hospice/hospital preferred), and EMR/EHR proficiency.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Alzheimer’s Disease and Related Dementias Training for Florida PACE patients per Florida Department of Elder Affairs.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and coordinates direct care with IDT for patient care planning and symptom management.
• Visits patients at home; conducts assessments and safety evaluations.
• Documents in EMR; communicates with IDT; initiates Plan of Care.
• Collaborates with physicians and facilities; supports referrals and transitions of care.
