Job summary

21st Century Home Health Services (21HHS) in Berkeley seeks a Part-time Registered Nurse Case Manager to manage SOC admissions and ongoing in-home care within a defined Berkeley territory. You will perform SOC assessments, follow-up visits, and coordinate with physicians and the clinical team to deliver high-quality patient care.

The role requires CA RN licensure, BLS/CPR certification, and a valid California driver’s license.

Qualifications

• CA RN license is required.
• BLS/CPR certification required.
• Valid CA Driver's License.
• Experience conducting Start of Care (SOC) and using OASIS is necessary.
• Experience with IV therapy is a plus.
• Experience collaborating with an interdisciplinary team is valued.

Responsibilities

• Perform Start of Care (SOC) admissions and OASIS documentation.
• Conduct follow-up visits and evaluation visits within territory.
• Make in-home visits to patients in designated Berkeley area.
• Reconcile medications and perform wound care as prescribed.
• Monitor patient conditions and report changes to Clinical team.
• Collaborate with physicians and staff to deliver quality care.
• Initiate and administer IV medications safely per protocol.
• Document observations and interventions in patient records.
