Job summary

21st Century Home Health Services (21HHS) is recruiting a Part Time Registered Nurse Case Manager for the Greater Sacramento area. You will assess home care needs, perform SOC admissions, and coordinate with physicians and staff to deliver quality patient care.

Requirements include CA RN license, BLS/CPR, CA Driver's License, and SOC/OASIS experience. Expect flexible schedules, mileage reimbursement, and comprehensive benefits. Field-based role with home visits in the Sacramento region.

Qualifications

• CA RN license (unrestricted) is required.
• BLS/CPR certification is required.
• Experience with Start of Care (SOC) and OASIS is highly desired.
• Strong home-health knowledge and high-acuity experience preferred.

Responsibilities

• Conduct Start of Care (SOC) admissions and OASIS as applicable.
• Perform follow-up and evaluation visits within the territory.
• Make home visits to patients in the designated territory.
• Document observations and patient care activities accurately.
• Initiate and administer IV medications safely per protocol.
