Job summary

Qualifications

• 3+ years of clinical practice in hospital, home health, or ambulatory care.
• Active and unrestricted RN license in NJ.
• NJ Choice certification completion is a contingency; an exam score of 80%+ is required.
• Willingness to travel 50–75% in Burlington County and surrounding counties.

Responsibilities

• Conduct home visits and comprehensive assessments for MLTSS and D-SNP members.
• Coordinate care with PCPs and care teams to develop cost-effective care plans.
• Document accurately in the member's electronic health record.
• Mentor new hires after proficiency is attained.
• Travel 50–75% across Burlington and surrounding counties.
