Job summary

CVS Health is seeking a Case Manager Registered Nurse (RN) to manage a caseload across South and Central New Jersey. The role involves in-person home assessments, coordinating multidisciplinary care plans, and mentoring new hires.

Ideal candidates have an active NJ RN license, 3+ years of clinical practice, and must reside in Camden County area. This full-time, remote-position requires substantial travel and benefits include healthcare and PTO.

Qualifications

• Associate's degree or nursing diploma required.
• Active and Unrestricted RN license in NJ required.
• Successful completion of the NJ Choice certification is a contingency of employment.
• 3+ years of clinical practice experience in a hospital or alternative care setting.

Responsibilities

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate the medical needs of members enrolled in MLTSS and D-SNP/FIDE.
• Facilitate requests for services such as adult medical daycare and personal care assistant.
• Schedule and attend interdisciplinary meetings advocating for members' safe discharge.
• Collaborate with members and care teams to develop care plans and authorize services.
• Document accurately and timely in the member's electronic health record.
• Coordinate care with members, authorized representatives, and care team participants.
• Use critical thinking skills to solve problems.
• Mentor new hires.
