Job summary

The Valley Health System in Southern Nevada seeks a registered nurse to lead discharge planning and utilization management efforts. You will work to ensure safe, timely discharges and optimal reimbursement while collaborating with patients, families, and the health care team.

Ideal candidates have a strong background in varied clinical settings and utilization review, with Nevada RN licensure and knowledge of post-discharge resources.

Qualifications

• RN graduate from an accredited school of nursing.
• Minimum three years experience in varied clinical settings.
• Two years experience in Utilization Review/Utilization Management or Case Management preferred.
• Knowledge of social and physical factors affecting discharge.
• Knowledge of community resources to meet post-discharge needs.

Responsibilities

• Establish safe, individualized discharge plans.
• Provide proficient, timely utilization management services.
• Collaborate with patient, family, and healthcare team to maximize outcomes.
• Identify and arrange post-discharge resources and follow-up care.
