Job summary

The Valley Health System in Southern Nevada is seeking an experienced Registered Nurse to lead discharge planning and utilization management. You will coordinate safe patient transitions, review cases, and collaborate with the care team to optimize outcomes and appropriate reimbursement.

Ideal candidates have 3+ years of varied clinical experience with a focus on utilization management, strong assessment skills, and knowledge of community resources to support post-discharge needs.

Qualifications

• Graduate of an accredited nursing program.
• Minimum 3 years’ clinical experience; 2 years in Utilization Review/Case Management preferred.
• Current Nevada RN license.
• Ability to assess social and physical factors affecting discharge and connect to resources.

Responsibilities

• Establish safe, individualized discharge plans and timely utilization management.
• Collaborate with patient, family and healthcare team to maximize reimbursement.
• Utilize data and workflows to ensure quality care outcomes.
