Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Minimum three years of clinical experience.
• Two years in Utilization Review/Utilization Management or Case Management preferred.
• Knowledge of discharge planning and community resources to meet post-discharge needs.
• Proficient in data collection and reporting.

Responsibilities

• Discharge patients safely and develop individualized discharge plans.
• Provide timely utilization management to maximize reimbursement for patient visits.
• Collaborate with patients, families and the healthcare team to coordinate care.
• Support continuity of care through post-discharge planning and resource linkage.
